| I have received a comment request for an image which I have not seen. I know I would have remembered this particular image as it has a very distinctive composition. I have responded to the author and commented anyway and explained the situation but how could this happen? I wanted to report this to Eric but can't seem to find any way to do this.

You can PM him.
Write to "admin" and give him the details of the image you didn't vote on, etc....

I get the occasional report of issues like this (it appears to be very rare, however). I have investigated the cause many times and instrumented code to double check the submission of votes and comments before committing them to record. However, the problem seems to persist as a ghost in the system. My belief is that the cause comes from an image being cached either at an ISP or within your local browser. Somehow this causes the wrong image to be displayed on either the vote or comment page.
My advice is to:
1) Clear your browser cache frequently
2) Use Firefox, if you are not already
I wish there was something I could do to completely eliminate this issue, but I believe I have made all the possible changes on the server, including sending instructions with each page to prevent caching (although, there is no way to guarantee a cache device honors this request).
I also want to be clear that this issue appears to be very rare with many users never experiencing the problem. You can continue to vote and comment with confidence.
